Nashville Office
In Nashville, Tennessee, most major medical insurance companies cover physical therapy. Physical therapy visits at this office are considered OUT OF NETWORK at this time. Therefore, insurance claims are not submitted and processed by this office and you will be responsible for submitting your claims for reimbursement. However, many of my patients are reimbursed 70-80% of the cost of treatment after their deductible is met and the claim forms are easy to complete. I have prepared a short list of questions (see below) to ask your insurance company so you can figure out how much reimbursement you will receive.
Questions to ask your insurance carrier:
- Do I have out of network physical therapy benefits?
- Do I need pre-authorization for treatment?
- What is my deductible for out of network benefits?
- What percentage of physical therapy will be covered?
- Do I have a maximum number of visits per calendar year or per condition?
- Do I need a referral from my primary care physician?
- How do I submit my claim for physical therapy?
